The UW-L – Gundersen – Mayo PA Program participates in the Centralized Application Services for Physician Assistants (CASPA). The CASPA application is easily completed online using their web-based data entry system. Applicants will submit all references and transcripts to CASPA for inclusion with their CASPA application. The UW-L – Gundersen – Mayo PA Program also requires submission of a Supplemental Application. This form is available for download using the links below. The Instructions, Supplemental Application, Sample Application and Application Checklist are downloadable as Acrobat Reader files (*.pdf). The files are optimized for use with Acrobat Reader 6.0 or higher which is available here. The Supplemental Application is easily completed using Acrobat Reader, but then must be printed and signed. It is then the paper copy of your application that is submitted to the program. Applicants are required to include an application fee of $50 with their application. The Supplemental Application for Summer 2014 entry is no longer available as the program's September 1 application deadline has passed. The application was posted on May 31, 2013 and available through September 1, 2013.
Submission of Graduate Review Exam (GRE) scores is required.
The program uses the following the code:
(New for 2013 application cycle) The program's CASPA GRE code is 0445. This code will result in ETS electronically forwarding results directly to CASPA for inclusion with the applicant's CASPA record.

Applicants do NOT need to apply for UW-L graduate admission until AFTER they have been offered admission to the PA Program (that is, unless they intend to take courses at UW-L prior to starting the PA Program). Graduate student slots are reserved for PA Program students at UW-L. If you meet the criteria for admission to the PA Program, you will almost certainly meet the criteria for UW-L graduate admission and admission to UW-L will be a simple formality.
