The UW-L – Gundersen – Mayo PA Program participates in the Centralized Application Services for Physician Assistants (CASPA). The CASPA application is easily completed online using their web-based data entry system. Applicants will submit all references and transcripts to CASPA for inclusion with their CASPA application. The UW-L – Gundersen – Mayo PA Program also requires submission of a Supplemental Application. This form is available for download using the links below. The Instructions, Supplemental Application, Sample Application and Application Checklist are downloadable as Acrobat Reader files (*.pdf). The files are optimized for use with Acrobat Reader 6.0 or higher which is available here. The Supplemental Application is easily completed using Acrobat Reader, but then must be printed and signed. It is then the paper copy of your application that is submitted to the program. Applicants are required to include an application fee of $40 with their application. The Supplemental Application for Summer 2013 entry will be posted on June 1, 2012 and remain available until September 1, 2012.
To assist applicants with completing the application a completed sample copy is available - Sample Application
Submission of Graduate Review Exam (GRE) scores is required.
The program uses the following the codes:
GRE School Code: 1914
Department/Major Code: 0609 (Medical
Sciences)
We ask that applicants make every effort to include both codes
when submitting requests with GRE. Failure to include the
Department/Major code will result in scores being misdirected
on campus and delay their arrival at the Health Science Center.
Applicants do NOT need to apply for UW-L graduate admission until AFTER they have been offered admission to the PA Program (that is, unless they intend to take courses at UW-L prior to starting the PA Program). Graduate student slots are reserved for PA Program students at UW-L. If you meet the criteria for admission to the PA Program, you will almost certainly meet the criteria for UW-L graduate admission and admission to UW-L will be a simple formality.
