Grant Transmittal Form

All proposals for extramural support must be accompanied by a UW-L Grant Transmittal Form. This form assures that appropriate officials review and give approval of each proposal in accordance with University and supporting agency regulations and it also provides the basic UW-L record of the processing transaction.

On-Campus Deadlines

The PI/PD should allow sufficient time for on-campus processing of proposals. Therefore:

  • Principal Investigator/Project Director (PI/PD) is strongly encouraged to discuss the budget with the Office of Sponsored Research, Scholarly, and Creative Activities (Grants Office), 145 Graff Main Hall, as early as possible but no later than ten (10) business days before the proposal mailing deadline.
  • The proposal should reach the Dean's or Director's Office in final form at least seven (7) working days before any sponsoring agency deadline.
  • This form duly signed by PI/PD, Chair/Director, and College Dean/ Division Director should be returned to Grants Office no later than five (5) business days before the deadline. The proposal summary and budget must be included with this form.
  • It is the responsibility of PI/PD to mail the proposal by the due date unless the proposal is to be transmitted electronically. For the latter, PI/PD must submit the proposal in electronic form to the Office of Sponsored Research, Scholarly and Creative Activities at least five (5) business days before the deadline.
  • A copy of the full proposal must be submitted within five (5) business days after the proposal submission.

The review responsibilities assigned to Deans/Directors and the Office of Sponsored Research, Scholarly and Creative Activities are of such a nature and extent that "walking a proposal through channels" will only be possible in the most exceptional circumstances.

All University employees who meet the federal definition of investigator must complete this Disclosure Form in order to comply with NSF (National Science Foundation) and HHS (Health and Human Services) requirements. An "investigator" means the principal investigator and any other person who is responsible for the design; conduct or reporting of federally funded research. Grant proposals cannot be submitted to federal agencies unless this form is completed, signed, and returned to your Dean and then the Office of Sponsored Research, Scholarly and Creative Activities. To assure that no grant proposal will be delayed, this form should accompany your proposal and Transmittal Form when you are seeking authorizing signatures. Review and Approval

Each proposal for extramural support must be approved by the Department Chairperson, the appropriate Dean or Director, the Office of Sponsored Research, Scholarly and Creative Activities before it is forwarded to the funding agency. Proposals which involve more than one department, school or college, or UW-System unit must be reviewed by appropriate officials of each. Proposals which involve animal use or human subjects must be approved by the appropriate University committees.

Principal Investigator

The terms "Principal Investigator" (PI) and "Project Director" (PD) are often used interchangeably to designate an individual who has the responsibility for seeing that proposals are submitted and that sponsored projects are carried out in compliance with the terms, conditions, and policies of both the funding agency and UW-L, including the submission of all required narrative and technical reports.

A project is generally conceived and carried out by the PI/PD who manages the funds entrusted to him/her to meet the project goals. As the person primarily responsible for the day-to-day management of a project, the PI/PD must be alert to both University and funding agency requirements.

Department Chairperson

The Department Chairperson shall:

  • review the proposal to assure that faculty and staff time commitments are reasonable and compatible with department workloads, present and planned;
  • determine that the percentages of time and the salaries are accurate; and
  • agree that the space, facility and service requirements are within the department's present capability if not specifically provided for in the proposal.

Dean or Director

The Dean or Director, or designated representative, shall review the proposal for completeness in accordance with Part III of these Guidelines. It should be confirmed that:

  • space, services and support requirements have been adequately provided for, and responsibility will be accepted for assuring the availability of matching (cost sharing) funds promised in the proposal; and
  • the project budget, salary rates, job titles and classifications are reasonable and appropriate.

In addition, it should be ascertained to what degree this proposal will commit the college or school to long-term support of project personnel or to a program which may evolve from the project.

Associate Vice Chancellor, Dr. Vijendra Agarwal

Under a policy revision passed by the Board of Regents of the University of Wisconsin System, the Chancellor or his/her designee(s) can enter into grant and contract arrangements on behalf of the institution.

As Chancellor’s designated representative, the Associate Vice Chancellor shall review the proposal for compatibility with the appropriate University plans, policies and regulations, as well as for conformity with agency requirements and policies.
