Curriculum Proposal Information (LX forms)

         Paper-based LX Form Processing - Contact the Records office at 785-8576 or records@uwlax.edu to request
                                                        LX 137, LX 138P, LX 138C, LX 139 and LX 140 forms. Include the type of       
                                                        equipment/software you use, i.e. Mac, Word95, Word 97, etc.

LX FORM DESCRIPTIONS

LX 137 - Summary of Proposed Changes
Summary form to be used when departments are proposing numerous changes.

LX 138 Forms
There are two LX 138 forms used to record curriculum changes - one for courses (LX 138C) and one for programs (LX 138P).

   LX 138P - Program Curriculum Change (yellow/goldenrod)
    Form used to make changes to majors, minors, concentrations, emphases and college core.  Changes include credits,
    required courses, electives, and other program requirements.  It is also used for submitting new program curricula to the
    curriculum committee.  Other committees that review new programs (e.g. - Academic Planning Committee)  may require
    different materials.  You may attach details instead of listing on form.

   LX 138C - Course Curriculum Change (yellow/goldenrod)
    Form used to make any change to a course including description, prerequisite(s), credits, title, grading pattern,  
    instructional pattern, course number or delivery options.  This form is also used to delete a course and to create a new
     course.  If proposing a new course, page 3 of the form must be completed.

     This form includes several check-off items to guide departments when considering various aspects of a course
     proposal/change.  

   LX 139 - Course Outline (green)
    Form used to provide course objectives, outline and requirements.  It is never used by itself; it accompanies the
    LX 138C. It must be submitted for all new courses.  Some changes submitted on an LX 138C might not require an LX 
    139; although it is logical that a LX 139 would be completed when changing the description, instructional pattern or
    number of credits.

LX 140 - General Education Course Change (green)
Form used to propose a course to be considered part of the General Education program or to make changes to an existing General Education course. It is a companion to the LX 138C.  This form does not need to be presented to the curriculum committee(s); it is for General Education Committee use.

LX 140D - General Education Course Deletion (green)
Form used to delete a course form the General Education program.  Submit LX 138C to permanently delete a course from the course catalog.
