Helpful Hints for LX Forms
Contact the Records office at 785-8576 or records@uwlax.edu to request the revised LX138C, 138P, 137, 139, and 140 forms. Include the type of equipment/software you use, i.e. Mac, Word95, Word 97, etc. The revised LX forms have been revised by Information Technology Services to correct the formatting errors.
When viewing the LX138C and 138P forms on the screen, the text will appear on the left side of the screen as one continuous column. The form will print in the correct format.
Filling out the LX forms:

Use the tab key to move to the fixed fields. If you need to back up, tab through to the bottom and back up to the top and through the form again. In some instances pressing the shift and tab key will back tab to the previous field.

You can access current catalog course descriptions from the web (UW-L home page – select current students, services for students, catalog undergraduate or graduate, catalog year, course descriptions)

You may choose to type the course description instead of cutting and pasting from the web, but by lifting the descriptions from the web, there is less chance for error.

If you choose to cut and paste course descriptions from the web, do the following:

Create a new document in Word (file-new)

Go to the web address as described in "b" above. Select course description, highlight, edit, and copy. Minimize the web document.

Go to the newly created Word document. Paste the web description here. Important ¾ save as a word document before you copy and paste to the LX form.

Open the LX form file.

Select Tools, unprotect document

Put the cursor in the area where the text is to be pasted and paste the description from the saved Word document into the LX form. This will allow you to underline new information or strike through deleted text by using format, font, etc. (When text is copied and pasted directly from the web, it is seen as unformatted text or HTML, and cannot be changed.)

Important After you have made your changes, select Tools, protect document - menu comes up automatically, select form and OK. This is an important step because the form could be corrupted if left unprotected. Use save as in a different file so template form is not corrupted.
Print LX138C and LX138P on yellow paper, LX139/140 back to back on green paper, and LX137 on white paper.
