Please be sure to return this order form no later than two weeks before the student’s birthday.  Also include your check and a birthday card if you please.  If you have further questions, please call the AMA Marketing Office at (608) 785-8097.  If there is no answer, please leave a message, and someone will return your call within 48 hours.  However, please take into consideration that it may take longer to return calls over weekends and breaks due to the fact that students are not on campus.
