UW-L MSDS Inventory Request Form

Use this form to request copies of MSDS's located in the UW-La Crosse MSDS Inventory. Please provide the Environmental Health and Safety Office with the chemical and manufacturer's name as they appear in the Inventory. All requests will be handled in a timely manner. Please send additional requests if you require more than five MSDS's. MSDS requests will only be processed for UW-La Crosse employees and students.
