Reporting Concerns

As required by the Animal Welfare Act (Policy 9.0109), a research facility must offer a method by which employees and concerned parties may request an investigation of any aspect of the animal care and use program.

If you wish to initiate an investigation by the IACUC, please complete the Request for Investigation form and it will be sent it to the IACUC Chairperson, Bradley Seebach (bseebach@uwlax.edu).    In order to preserve confidentiality, your request will be viewed in its entirety only by the Chairperson and your name will be removed from the form prior to it being shared with anyone to conduct the appropriate investigation. You will not be subject to any retaliation for expressing your concern.
