Equal Employment Opportunity Form

The Equal Employment Opportunity Form (EEO form) must be included when mailing the letter of acknowledgement and confidentiality request form to all those who have applied for a faculty, academic staff, or administrative position. Please phone the Affirmative Action & Diversity office at 785-8541 to receive copies of the EEO form. Do not duplicate your own forms. They must be printed on special weight paper due to Postal Service requirements.

Candidate/applicant numbers must be included on each EEO form that is mailed so that the AA&D office can match returned EEO forms to specific applicants. If a form is returned without a candidate number, you will be notified by AA&D to send the applicant another EEO form that includes the number.
