CBC Procedures:

The CBC must be completed as a part of the hiring process before an official offer of employment is given to a candidate.

The information on the New Athletics Employee Information form (for EOP only appointments) or the Personnel Action Form (for contract appointments) gives human resources the information they need to initiate the electronic CBC for a candidate.

When the CBC consent has been completed by the candidate and the results of the CBC have come back, an EOP may be generated for the employee by the department or a contract may be generated by HR.

Extraordinary payments (EOPs) are used to pay employees for teaching overload classes or additional work that unusual, short-term or non-reoccurring in nature and is done beyond their regular duties. EOPs are also used to pay short-term, non-contracted employees for performing specific duties.

The EOP should not be used for Inter-institutional agreements (IIA) or Payments to Individuals (PIR).

IIA is used to pay an employee of another UW institution amounts greater than $1,000. It is necessary to have the employee complete a separate Federal I-9 employment eligibility form before they complete work for each UW institution at which they are employed. (No other payroll paperwork is required)

PIR is used to request payments to individuals who are not employees of UW-L or another UW institution and the work being performed is not conducive to an employer/employee relationship (i.e. the institution is not in control how the work must be done, only the result of the work). Payroll Paperwork

All new employees and employees who have not been active on the UW-L payroll in the past 12 months need to complete the required payroll paperwork including Federal I-9, W-4, Direct Deposit, and Self Identification survey. Click here for links to these forms.
Information for New Employees

As UW System employee, you are required to report incidents of child abuse or neglect to the police or to a local child protective services agency. Click here for the Governor's executive order and an FAQ about the responsibility held by UW System employees.

Use the My UW System portal for accessing your earnings statements and annual tax statements. You will have access to the portal and access to a UW-L email account for the duration of time that you are employed. Contact the Human Resources at 785-8013 or the Eagle Help Desk at 785-8774 for assistance with email and portal access.
