Annual W-2 Tax Statements Available on the My UW System Portal
Current employees can now log in to the My UW System portal to view or print their 2013 W-2 statement at their convenience. W-2's will also be mailed to the employee's home address (current and former employees) during the last week of January. Please contact the Human Resources office with any questions regarding tax statements.

TIAA-CREF on Campus A representative from TIAA-CREF, a vendor of the UW System Tax-Sheltered Annuity 403(b) program, will be on campus on Tuesday, Wednesday and Thursday, April 15, 16, and 17 in Cartwright Center. Individual counseling sessions can be scheduled by contacting TIAA-CREF directly. For details and contact information, please see the TIAA-CREF flyer.
Fidelity on Campus A representative from Fidelity Investments, a vendor of the UW System Tax-Sheltered Annuity 403(b) program, will be on campus on Tuesday, April 15 and Wednesday, June 11 in Cartwright Center. Tax-Sheltered Annuity Workshop Invest in your future! Rob Wolf and Cedric Steine will present a workshop with information about investing the UW System Tax-Sheltered Annuity 403(b) program on April 17 at 2:30 pm in Cartwright Center. See the TSA workshop flyer for more information.
TIAA-CREF Lunch and Learn Presentation TIAA-CREF, a vendor of the UW System Tax-Sheltered Annuity 403(b) program, is hosting a lunch and learn presentation titled The Starting Line: Why retirement saving should begin now on May 21 in Cartwright Center. Register for the event by contacting Cedric Steine in Human Resources, a complimentary boxed lunch will be provided to those who register. Dental Network for HMO Members The State of Wisconsin Health Plans, with the exception of the Standard Plan and State Maintenance Plan, now have a uniform dental benefit that became effective January 1, 2014. The new benefit is designed to provide coverage both in and out of network but at different levels of coverage. For preventative and diagnostic services, in network is covered at 100% and out of network provides 75% coverage up to $1000/member/year.
Recently, some individuals covered by Health Tradition, received a bill for dental services that they thought would be fully covered. In order to ensure that your dental services are fully covered, it is important that you see a provider in the Health Tradition Dental Network. The Gundersen Health Plan does not have a dental provider network at this time.
Please refer to the left column of the Dental Comparison Chart for additional information. Be aware that if you choose to go to a dental provider out of network, Health Tradition will only cover 75% of the cost for preventive and diagnostic dental services.
WRS Small Group Retirement Sessions Employees considering retirement within the next 12 months or who are within one year of minimum retirement age are invited to attend. The sessions are April 2 at 1:30 pm and April 3 at 9:30 am. The sessions fill up quickly, and employees are encouraged to make their appointment as soon as possible. Emergency Contact Information Is your Emergency Contact Information up to date? This information is imperative as we have had the need arise to contact a designee only to find no one is listed. When medical or other emergencies come up, every minute counts. Please help us help you out should the need ever arise. You can update your information on your My UW System portal by clicking the "update my personal information" link or you may complete and return the emergency contact form to the Human Resources office.
Open Meetings on Google Calendar To submit a meeting notice or view upcoming meetings, visit uwlax.edu/meetings. For search and screen, promotion, retention, tenure, and other employee evaluation related meetings, visit uwlax.edu/hr/post.htm. To post a search and screen open meeting notice, email email@example.com at least 24 hours in advance of the meeting.
Mandatory Reporting Requirements The Governor recently signed into law Executive Order #54 which declares that all employees (faculty, staff, administrators, coaches, and student employees) of the University of Wisconsin System are required to immediately report any case of child abuse or child neglect that they learn of in the course of their employment. Reports of child abuse or neglect under Executive Order #54 should be made to University Police. Click here for Executive Order #54 information and training resources.
