AUTHORIZATION TO RECRUIT  (FRONT OF THE "PINK SHEET")

Sections 1 and 2 are to be completed by the department or unit recruiting for the open position.

 

 

SECTION 1:       BUDGET/POSITION INFORMATION: Complete the budget information requested.  Questions regarding budget, fund sources, or budgeted salaries should be referred to the Dean or Provost/Vice Chancellor.

 

Dept./College Name--Name of your specific department and college.

 

DDU Number and IBAC--Found in the University Chart of Accounts, on the mainframe using trancode ACCDDU, or by contacting the Office of University Financial Administration.  DDU stands for Division, Department and Unit, and IBAC for Interim Budget Activity Code.

 

Recruitment Number--Numbers to be provided by dean/division officer (check with Human Resources to avoid duplicate numbers).  The recruitment number consists of 7 digits; the first two indicate the year the hire will occur, i.e., recruiting initiated for the academic year 2003-2004 would start with 04. The next three places indicate the department (math would be mth).  The final two digits distinguish this search from all other searches and are sequential.  The first recruitment for the year would be 01, the next 02, etc.

 

Fund Source--Specify funding source, indicate on the appropriate blank line.  Usually General Purpose Revenue (GPR), which is coded as “102”.  Please consult Human Resources, if you have any questions.

 

Employment Period-- Generally, faculty will be employed on an academic year basis.  Specify beginning and ending dates if other than dates listed.   

 

% FTE Percentage of Full Time Equivalency (Generally 100%).

 

Budgeted Salary Range--Dollar amount budgeted for a specific time period, e.g., $25,000 to  $34,000 per academic year.  You can see past budgeted dollars allotted on your B-5.

 

New Position or Replacement--Check whether it is a new position or a replacement and specify the employee who is being replaced.

 

Title--Check the correct title, either professor, associate professor, assistant professor or instructor.  Multiple titles may be checked if the hire could be either assistant or associate professor based on credentials and experience.

 

Working Title--Rarely used; however some faculty have working titles, such as Director of Academics (not an actual title, just an example).  If you have any questions, please call Human Resources.

 

SECTION 2:       DEPARTMENT/COLLEGE SIGNATURES:  Signature of the Chair indicates approval to proceed with the established recruitment procedure.

 

SECTION 3:       PROVOST/VICE CHANCELLOR SIGNATURE: Final approval is given by the Provost/Vice Chancellor who signs and forwards to Human Resources
