DIRECTIONS FOR COMPLETING THE PERSONNEL ACTION FORM (L-1347) / BACK OF THE "PINK SHEET"

Approval must be received from both the provost and assistant to the chancellor for affirmative action & diversity before an offer of employment is extended.

The Personnel Action Form is completed by the dean or designee (such as the department chairperson).  It is the dean’s responsibility to ensure that all entries are correct.         

SECTION 4: FACULTY HIRING/APPOINTMENT TYPE

     Recruitment Number--The number assigned for this recruitment by the department.  It can also be found on the upper right hand corner of the front of this form (Authorization to Recruit).

      Name of the Applicant--As shown on social security card.  Include middle initial.

S.S.#--Social security number.

Phone Number and Mailing Address--This is the address where the employment contract will be sent.

Campus Address--The room number, building name and phone extension that have been assigned.

Sex--Indicate gender.

Terminal Degree--Yes or no.  Based on criteria in the Faculty Promotion Rules.  

Education--Indicate highest educational level attained.  If the appointee has a doctorate, please enter the month and year it was earned, as evidenced by official transcripts.  

Years of Credited Prior Experience--Indicate years of prior work experience credited to the appointee.  (Used for promotion eligibility)

Probationary Status--If probation is checked, indicate the number of years on probation or check tenure (for those hired WITH tenure).

Actual Employment Dates--Most faculty will be hired for the academic year so the first box will be checked.  Those hired for second semester will have the second box checked.  For any other option, please explain.  Human Resources may be consulted, should you have a question.

Faculty Title Code--Check the appropriate box; professor, associate professor, assistant professor or instructor (rarely used).

Working Title--Indicate a working title for the position, if applicable.  This is rarely used.

% FTE--Full time equivalency is generally 100% for faculty.

    Actual Salary--Indicate actual starting salary.

Salary Basis—The salary basis for most faculty will be on an academic year. If other, please specify.

Fund, IBAC, 1st DDU, %--The full account code where the salary and fringe benefits should be charged.  Also, indicate the percent of salary to be charged to this account.  Under normal circumstances, this account should be the same as on the front side of the pink form (Authorization to Recruit).

Fund, IBAC, 2nd DDU, %--The second account code if salary is to be split between different accounts.

Dept--indicate the name of the department such as Math.

SECTION 5.  SIGNATURE OF DEAN:

Dean--Signature of the dean indicates offer of employment has been approved, in consultation with provost (or chancellor), the assistant to the chancellor for affirmative action & diversity, and offer of employment has been extended and verbally accepted.  The completed form will be reviewed for consistency by the Office of Human Resources and a contract prepared from the information provided.

SECTION 6.  PROVOST/VICE CHANCELLOR:   The Provost/Vice Chancellor signs all faculty pink sheets.
