REPORTING CONCERNS
As
required by the Animal Welfare Act (Policy 9.0109), a research
facility must offer a method by which employees and concerned
parties may request an investigation of any aspect of the animal
care and use program.
If you wish to initiate an investigation by the IACUC, please
complete this form
and send it to the
IACUC Chairperson. In order to preserve
confidentiality, your request will be retyped by the
Chairperson, without your name, for use during the
investigation. You will not be subject to any retaliation
for expressing your concern.
