Introduction

Welcome to the Health Science Center. The University of Wisconsin-La Crosse Physical Plant, one of your HSC Consortium partners, holds the contract for building services in this facility. We have produced a guide to explain the way in which we propose to administer this contract. This guide will answer many of the questions we have been asked so far and help you determine who to call for physical plant services at the HSC. Eventually there will be a system of teams in place to receive a variety of requests from the building occupants. These teams will study those requests and recommend solutions. Those requiring review at a higher level will be presented to HSC, Inc. by the respective team(s). A calling list and floor plans are attached to this guide for your convenience. We intend to provide the quantity and quality of services you need to fulfill your mission to our students and the public. We face a formidable task, because the HSC is new and this situation is new for all involved. As we learn from our experiences here, please help us to improve the quality of our services by regularly sending us your comments and suggestions to dahlj@westerntc.edu. Most of these services will be provided behind the scenes or after hours. You may be very interested in the workings of some of these services and others may not interest you at all. To give you a more complete understanding of the functioning of the HSC, however, a few words will be said about each. As you read through these descriptions, please jot down any questions you may have and send them to us. Service Descriptions

1) Audio-Visual Equipment, Multimedia Development, and Distance Education

  • Educational Technologies provides training, equipment, and support for the audio and video technology in the building. General training sessions are scheduled periodically and personalized training is available upon request. Call 785-5145 to report any problems or request assistance.

  • Five mobile technology carts equipped with a computer, projector, visualizer, VCR, and speakers are available for use within the building. Various additional equipment such as TV/VCR carts and slide projectors is also available.

  • Room 2056 is the multimedia development area. Computer equipment, scanners, printers, digital cameras, and other equipment are available for use by faculty and staff members. Equipment can be checked out but is not allowed to leave the building. The Educational Technologies staff is available to assist with any development needs.

  • Audio and video can be sent to nearly every room in the building from the master control room on the second floor. This includes satellite transmissions, DVDs, cassette tapes, and other forms of technology. Call 785-5145 for more information.

  • There are three designated distance education (DE) rooms and one portable DE unit in the building. Educational Technologies schedules all DE sessions and provides DE site support. Specialized training is offered at the beginning of each semester and is also available upon request.

2) Carpentry (Adjustments, Repair)

  • Please report any carpentry requests (examples: hang items on walls, construct shelving, etc.) to the
    UW-L Physical Plant at 785-8585
    After review, we will make the required adjustments in a timely manner. Work order forms can be obtained from the UW-L Physical Plant.

  • Requests for major repairs or alterations will be studied by the HSC Physical Facilities Team and must be approved by HSC Inc. before being scheduled and implemented.

3)     Charge Back Policy

       HSC INC. FINANCIAL AND ADMINISTRATIVE POLICY FOR DEPARTMENTAL
     CHARGE BACKS

           

         HSC Inc. is responsible to insure the Health Science Center is maintained in

        accordance with all applicable Wisconsin Building Codes.  HSC Inc. will

        provide all routine maintenance and repair services for the Health Science

        Center and all facility equipment owned by HSC Inc.  This includes tasks

        such as repairing plumbing leaks and electrical problems; Heating, Ventilating,

        and Air Conditioning maintenance; structural maintenance including painting,

       repairs to walls, ceilings, roof, etc.; and providing routine custodial and

        grounds services. Equipment that is considered as being owned by HSC Inc.

        includes general classroom furnishings/equipment, public lounge equipment/

        conference room furnishings, etc.

 

            The following list identifies those services that are not included in the

            routine maintenance and repair services and are not covered by the HSC Inc.

            operating budget.  This is a comprehensive list, but not all-inclusive, of

            services to be charged back to academic departments/programs and other areas

             making requests for these types of services.

 

           

1.  Replacement, repairs, refinishing, servicing and reupholstering of

            furniture and equipment used primarily by a department or program.  This

            includes office furniture and office equipment, department/program supplied

            laboratory furniture and equipment and any other special hardware of apparatus.

            Excludes classroom and general use furniture and equipment not assignable to any

            department or program.

                          
            2.  Maintenance of science and lab tables and hardware. Maintenance on

            special lab fixed equipment such as environmental chambers, freezers,

            refrigerators, ice makers, water softeners, de-ionizers, acid dilution systems,

            compressed air, water distillation systems, autoclaves, etc. that is provided

            by and owned by a department/program.

 

            3.  Installation, maintenance and replacement of laboratory

            apparatus,  special laboratory plumbing, special HVAC (such as A/C units installed

            and used for computer labs) and special electrical requirements to accommodate

            additional equipment installation or change in room use.

 

            4.  Maintenance and installation of departmental/program kitchen equipment,

            appliances and other department-owned apparatus and moveable equipment.

 

            5.  Key cutting (except for initial supply of keys) and re-keying of

            functionally sound locks as well as the purchase and installation of

            new security systems.

 

            6.  Replacement of departmentally installed carpeting.

 

            7.  Replacement of carpeting supplied with the building other than for wear.

 

            8.  Installation of department/program automatic door closers or card access

            systems where none exist.

 

            9.  Installation, changes and repair of office and desk signs, pictures,

            projection screens, marker boards, tack boards, mirrors, etc.  in offices.

 

            10.  Any change, renovation or remodeling in the building structure or in

            its hardware or its mechanical or electrical systems when requested by a

            department/program.

 

            11.  Special departmental request for services outside the timeframe of

            routine maintenance, such as the repainting of rooms more frequently than

            the institution's schedule. (This includes classrooms, labs, hallways,

            offices, department/program rooms and general use areas).

 

 

            12.  Replacement of glass in doors, windows, or partition walls when that

            present glass is functionally sound, i.e., change clear partition glass

            to frosted glass, etc.

 

            13.  Remodeling and/or installation of shelving, cabinets and furniture and

            relocation of these items when requested by a department/program.

 

            14.  Changing window treatment supplied with the building other than for

            wear (e.g., changing from blinds to draperies, etc.)

 

            15.  All material and labor costs, including overtime, for services (setup,

            teardown and cleanup) performed for GPR departments in support of special

            programs such as summer camps, workshops, or any other activities.ivities.

 

            16.  All material and labor costs associated with repairs due to negligence

            or willful damage to the Facility or equipment owned by HSC Inc.

 

            Occupants must have HSC Inc. approval to make any changes/modifications to

            the existing Facility. The department/program requesting a change must make

            arrangements for such changes through the current vendor providing the

            Operations & Maintenance Service for HSC Inc.   The O&M vendor will

            determine if the request requires HSC Inc. Board approval and coordinate that approval. 

            If the O&M vendor determines Board approval is not required, the vendor will

            then facilitate the building modification. If an outside vendor is being

            considered for such work, plans and specifications will need to be submitted

            to insure compliance with Wisconsin Division of Facility Development Construction

            Specifications.

 

4) Computer Services

  • If you have a computer or network related problem between the hours of 7 a.m. and 4 p.m., please call the WTC Help Desk at 789-6254.

  • If it is after 4 p.m., you may contact the WTC Academic Computer Lab support person at 785-9703. If you do not get an answer, please dial 789-1237 to page the on-duty support person.
     
  • There is a computer lab in the Health Science Center (Room 2030) for student use. 
    Hours of operation during the fall & spring semesters are:

5) Copiers

  • Faculty copiers are located on fourth floor and are accessible by using an access code. To obtain an access code, contact Julie Dahl at WTC.

  • A coin-operated student copier is located in the basement student lounge area.

6) Dental Clinic

  • For questions or course information related to the WTC Dental Hygiene Program, call Program Director Antonio Re at 785-9160.

  • For questions related to the Three Rivers Community Dental Clinic, call 789-6200.

6a) Electrical (Automated Lighting System)

  • The building has an automated (computerized) lighting system that regulates the hours that lights are on in the building. The system is set to shut lights off at 11 p.m. Monday through Friday. Anyone working late in the building can override the system by pressing "up" on the black override buttons located in various rooms and hallways throughout the building.

7) Electrical Service (Utility)

  • Electricity is supplied to the building by Excel Energy, Inc.

  • The HSC is equipped with a standby generator that runs on natural gas, which is capable of providing enough power to run emergency lighting, some environmental equipment, elevators, and critical research coolers for an indefinite period.

8) Elevator Service

  • If you are in the elevator and experience a problem, please pick up the telephone found inside the elevator car. This will connect you immediately with dispatch that will respond to your call.

  • If you notice an operational problem with any elevator, please call the WTC Physical Plant at 785-9109.

9) Financial Services

  • Western Wisconsin Technical College (WTC) has been appointed as the Fiscal Manager for the Health Science Center (HSC) Building. A budget has been prepared by HSC, Inc. Any invoices relating to services provided by the HSC in the facility are to be approved by the appropriate oversight agency and sent to the WTC Business Office for payment. The expenses of operating the HSC Facility will quarterly be billed back to the occupying organizations, proportionately, based upon the total square feet in the facility they utilize.

  • THERE ARE DIFFERENT KINDS OF EXPENDITURES RELATED TO THE BUILDING AND ACTIVITIES IN THE FACILITY. THE RESPONSIBILITY FOR THE EXPENDITURE DEPENDS ON WHETHER IT IS A PARTICIPATING ORGANIZATION EXPENDITURE OR AN HSC PROVIDED SERVICE EXPENDITURE.

  • THE GENERAL RULE IS THAT WTC, UWL, READ CLINIC, DENTAL CLINIC, STUDENT HEALTH SERVICE AND ANY OTHER AGENCY OR ORGANIZATION OCCUPYING SPACE IN THE HSC FACIITY ARE EXPECTED TO COVER ANY EXPENSES ASSOCIATED WITH THE OPERATION OF THEIR PROGRAMS IN THE FACILITY.

  • THE ONLY EXPENSES BILLED TO THE HSC BUILDING ACCOUNT WILL BE GENERAL OVERHEAD EXPENSES TO OPERATE THE FACILITY (heat, lights, maintenance, custodial, building management, AV Services, security, parking and any other expense related to shared facilities).

10) Grounds Upkeep (Snow Plowing/Removal, Lawn Care)

  • UW-L Landscape Services is responsible for all lawn care services.

  • Snow will be plowed and hauled away from the lot immediately after most snowfalls, unless classes are not in session. At those times the snow removal vendor may be allowed to schedule removal when it is convenient for them. In no case will snow be allowed to remain more than a few days.

  • UW-L crews will routinely remove snow from city sidewalks.

11) Hours of Operation

  • The building is open from 7:00 a.m.- 10 p.m. Monday through Friday from late August through mid-May.

    Hours for summer and holiday breaks are subject to change. Security will unlock the   building as needed on weekends. Students must be supervised or have faculty permission to be in the building unsupervised. Faculty are responsible for notifying UW-L University Police,  785-8000, of these special access needs at least 48 hours in advance.

12) Housekeeping (Cleaning, General Maintenance)

  • All cleaning personnel are required to wear uniforms or identification nametags so that they can be easily identified. UW-L provides the building's housekeeping and janitorial service.

     

  • Two daytime and two evening custodians will be in the HSC Monday through  Friday. These custodians will routinely shovel snow from doorways, spread  ice-melter, clean up spills, help with rubbish removal, restock restroom  and locker room supplies, provide routine cleaning, and conduct  special cleaning.
     

  • Routine cleaning will include all surfaces, except desktops, lab tables and horizontal surfaces that have   documents upon them. Papers found on  copiers, printers, or tables will be placed in the lost and found. Trash will be removed daily. 

    There is a communication logbook in the fourth floor mailroom in which you can leave message for the  housekeepers.  If you need to contact a custodian or report a custodial problem, contact Ann Peterson at 789-6172.  If she is not available, call the custodial cell phone at 386-2518. 

         UW-L Custodial Services may also be contacted at 785-8912 if the first two options do not respond.

         Custodians will not clean within the lower level research areas, the fifth floor research labs, the Bio-
         Safety  Level 3 Lab on fifth floor, or other hazardous spaces, unless requested by the area's staff
         and authorized by UW-L Custodial Services. Special cleaning must be scheduled in advance.

 

13) IN CASE OF EMERGENCY

       Call 911 - Life Threatening

       Provide follow-up call to University Police at 789-9999.

       In case of fire, evacuate the building. When the fire alarm system is  activated, a signal is sent to the
       UW-L  Switchboard and they call 911.

       Wait outside the building until instructed by Fire Department or a Police Officer to return. 
       DO NOT GO BACK  INSIDE without this confirmation.

       Always evacuate the building whenever the alarm is activated.

14) Keys and Access Cards

  • All HSC staff will receive keys for their individual offices, classrooms, labs and shared areas. Staff members who need to enter the HSC after hours or who work in the lower level research area or the Bio-safety Level 3 Lab on the fifth floor will also be issued access cards.

  • ALL cards and keys will be registered to individual staff members by use of a key request form. You are responsible for the keys and cards that have been assigned to you. Keep them in your personal possession. Do not lend them out. If someone needs access to a space and has forgotten their key, a clerical staff member or custodian will be dispatched to unlock and lock doors as required.

  • Immediately report any destroyed, lost or stolen keys and access cards to Ann Peterson.

  • Touch pad access is installed on doors to Rooms 0086, 1144, 3002, 3014, 3026, 3048, 3051, 40002, 4022 and 5092. Codes are issued by the appropriate faculty.

  • If you are leaving employment in the Health Science Center, turn your keys in to your immediate supervisor.

  • If you are locked out of a room, please stop by Room 4031 or 2056 to gain access.

  • Strict compliance with the key procedure is the first step to assuring adequate building security for us all. We need your cooperation. Thank you.

15) Life Safety Systems (Fire Extinguishers, Sprinkler System, Alarms)

  • Life Safety Systems (Fire Extinguishers, Sprinkler System, Alarms)  Service will be provided by the
    UW-L Physical Plant. Please report any missing fire extinguishers, burned-out exit lights, or other malfunctioning  safety equipment.       

    Please contact the UW-L Physical Plant for assistance with  maintenance-related issues.

     

  • Please remember that all parts of the HSC are protected by automatic fire detection and suppression (fire sprinkler) systems.

  • Floor plans showing emergency egress routes will be posted on each floor.

  • In the event of a fire the elevators automatically shut down and return to the first floor, so it is always preferable to use the stairs in an emergency.

16) Lost and Found

  • See Ann Peterson in the first floor lobby.

17) Mail (UPS, Fed Ex, First Class, Interoffice)

  • Interoffice, first-class mail, and parcels originating in the mail department of UW-L will be delivered and picked up twice a day by UW-L to the mailroom on the fourth floor. Please direct your questions to Deb Veglahn at 785-6494.

  • Interoffice, first-class mail, and parcels originating in the mail department of WTC will be delivered and picked up twice a day by WTC to the mailroom on the fourth floor. Please direct your questions to Monique Leslie at 785-9183.

  • Large parcels or freight will be received at the HSC. Drivers will be directed by signs to call WTC for assistance. We will then dispatch a custodian to receive the freight. They will sign for the item and deliver the paperwork to clerical staff on the fourth floor. (Direct the vendor to call 785-9109 for delivery arrangements to save time.)

  • A signature is required of the person who placed the order. Clerical workers may sign on behalf of unavailable staff.

  • Please assist us in getting large packages/freight properly delivered by using this ship-to address: Health Science Center, 1300 Badger Street, La Crosse, Wisconsin 54601, Attention: Mr./ Ms. (Ordering Person), Room No. _____.

18) Office Supplies

  • Office supplies are the responsibility of the individual department or program.

19) Parking (Meters, Ticketing, Lot Maintenance)

  •  UW-L Protective Services is responsible for the USC parking lot.  Officers 
    and ticketers will provide enforcement between the hours of 8 a.m. and 4 p.m.,
    Monday through Friday.  Parking is not enforced on weekends.  There is no overnight parking, skateboarding, or extended parking.

    Visitor permits are available from individual programs at a fee of $2   per day per permit.  The permits may be issued to  patients, guest  lecturers, volunteers for labs and classes, advisory board members,         program sponsored special events, short term trainees, vendors, repair  and employees.  Visitors with permits should use a hooded parking meter.  Visitor permit questions can be answered by calling Ann        Peterson at 789-6172.

    Due to limited parking availability, HSC faculty, students and staff are encouraged to
    walk, ride bike or use the MTU bus. If desired, faculty, staff and students can  purchase parking in the metered spaces of the HSC parking lot.   Limited parking will be available in Lot #14 and Oakland and Badger Streets.   This is a UW-L lot and UW-L permits are required.  See Ann Peterson for details.

    MTU busses stop at the Health Science Center.  WTC staff can use the
    MTU busses free with a WTC ID. Bus schedules can be obtained in the first floor lobby.

     Loading and unloading are allowed either in the circular drive or near the service elevator on the west side of the building. Vehicles involved  in loading and unloading are to be moved out of these areas immediately when finished. Carts for this purpose can be obtained in the first floor  lobby using your faculty key.  Contact Ann Peterson, 789-6172, for more information.

     

20) R.E.A.D. Clinic

  • Call Carol Kirk or Cyndy Patros at 784-8665 for more information on the R.E.A.D. clinic operation.

21) Radiation Safety

  • Call Mary Ellen Jafari at 782-7300 to discuss issues related to radiation equipment and radioactive materials in the Health Science Center.

22) Room Rental & Room Schedule

23) Security (Rounds, Inspections, Record Keeping)

  • A random Security Patrol of the facility is provided year-round, but is focused toward the evening hours. This patrol is provided by uniformed UW-L Police.

  • Officers can be contacted by calling 789-9999 - 24 hours a day. Incident investigation and reporting, along with Crime Prevention Activities, are also provided.

  • Security telephones are located on each floor.

24) Steam and Chilled Water Service (Utility )

  • The steam used to heat and the chilled water used to cool the HSC are supplied by tunnel from the central UW-L plant.

  • Regular one- or two-week shutdowns are periodically scheduled by UW-L. These lapses in service allow for maintenance of UW-L equipment. We are usually notified several weeks in advance and will, in turn, notify you. We also will do whatever we can to minimize any inconvenience to you.

25) Student Health Center

  • Call 785-8558 or 785-5101 for access to services. The Student Health
    Center provides comprehensive primary care services for WTC and UW-L Students.
    Services include primary care for illness or injury, care for chronic health problems, minor surgical procedures. medical management of mental health concerns, obstetric (limited) exams and gynecological services, immunization and TB testing, physical exams, reproductive services, basic lab services, allergy injections, HIV testing and  counseling, physical therapy services, consultation and referral. All UW-L students are eligible to use the Student Health Center. The fee is  automatically included in the tuition statement for students who are  registered for a minimum of seven credits during the fall and spring        semesters or a minimum of three credits during the summer session. WTC students enrolled for six credits or more in Fall and Spring Semester are eligible to use the Student Health Center. Cost of this service is provided by student activity fee money. There is a $10.00 co-payment, per visit, for WTC students eligible to use the Student Health Center.  Some servicesare billed directly to the students. Please contact the WTC Student Life Office at  785-9445 for additional eligibility guidelines.

     

26) Systems Maintenance (Non-Instructional Building Equipment Upkeep)

  • Please assist us by promptly reporting any failures of which you are aware. Email reports are preferred so that a written record is produced for service follow-up. Examples of systems failures are heating and cooling problems, electrical power interruptions, plumbing problems.

  • Please e-mail notices of systems failures to petersona@westerntc.edu or call 789-6172.

27) Telephone Service (Utility)

  • Local and long distance telephone service is provided by Century Telephone.

  • Conference call arrangements should be scheduled through contacting Terry Smith or Dave Harris at 785-5145.

  • The HSC, UW-L, and WTC are provided with Centrex-type service. This means all our phones are actually hosted by the main switch in the telephone company's central office. We can dial each other without dialing the 8 prefix.  The last five digits are all that's necessary.

  • If you need to order a new telephone, change phone numbers, or schedule any other telephone-related service, please contact Diane Yehle (WTC - 785-9557) or Hedy Otto (UW-L - 785-8002).

  • Outside lines are accessed by dialing an 8 and waiting for dial tone. The exception to this procedure is when dialing 911; an 8 is not required in this instance.

28) Tours

  • Information regarding tours of the building is available by contacting Ann Peterson at 789-6172

29) Waste Management (recycling, general rubbish, medical/hazardous)

  • Routine building waste and rubbish will be collected by custodians and placed in a Waste Management dumpster located southeast of the receiving area of the HSC.

  • Medical waste will be collected from labs by the daytime custodians and stored in the receiving area until it is removed by Stericycle (formerly BFI). Collection will be based upon the normal routine by which Stericycle serves some of our local hospitals.

  • A paper recycling program is in place. Wastebaskets are provided for recyclable paper alongside traditional rubbish containers. If you need a container, please contact the UW-L Custodial Services at 785-8912.

30) Window Cleaning

  • Most daily window washing will be provided by the custodians; however, at least once a year a vendor will be hired to clean all windows in the HSC

Conclusion

The HSC is a remarkable place in which to work. Its construction is very high quality, and the equipment and furnishings are first-rate. Building services will be constantly monitored for quality. We are hoping that these factors among others will prompt you to do your part to keep the HSC a premier facility:

  • All consortium faculty and staff who work in the HSC can park in the lots located at their sponsoring institution. Please walk or ride the bus from there to the HSC. If desired, faculty, staff and students can park in the metered spaces of the HSC parking lot.
  • Do not make holes in the walls. Contact the UW-L Physical Plant for assistance.
  • Do not paint your walls. Repainting will be done for you as necessary.
  • Do not use open flame, such as lighted candles, in your work area.
  • Do not clutter common areas with personal items or decorations. Keep them within your own workspace and below the tops of enclosing cubicle walls.
  • Do not post announcements on glass or painted surfaces. Please tell students to post their approved announcements on the designated bulletin boards.
  • Do not pile materials on the floor. It makes cleaning more difficult.
  • Do not use tobacco products within the HSC or closer than 20 feet to a building entrance doorway.
  • Do not lend your access card or keys to anyone.
  • If you work after hours, please make sure the door is latched and locked when you leave.
  • Please ask us if you are unsure about what to do in a given situation. If we do not know the answer, we will refer the question to the proper team or the HSC Inc. for discussion. Decisions will be based upon what is most fair to all concerned.
