Certificate Program: Application Forms

  1. Submit a UW-La Crosse non-degree application. This can be completed online at the following address: https://apply.wisconsin.edu.  Select Certificate at the Undergraduate level and Health Related Studies: Medical Dosimetry (Certificate Only [SAH]), where applicable.
  2. Official transcripts of all college and university work must be submitted to:

  3. Submit the UW-L Medical Dosimetry Certificate Application and all supplemental materials together by February 1st for general admission including the non-refundable $50  program application fee payable to the University of Wisconsin - La Crosse to:

Note: Failure to receive your application materials by February 1st will result in your application not being reviewed.

Track 1 - RTT

  1. Proof of current ARRT certification
  2. Four letters of recommendation; (submitted with application; they must be in sealed envelopes with the signature of the reference across the seal)
  3. Observation Rotation: all applicants must have completed a 40 hour observation in medical dosimetry. Read the observation information and associated forms that are due as part of the application process: Clinical Observation Forms
  4. Application Checklist

Track 2 - OJT

(You must be working in medical dosimetry as an "on the job training" individual at your facility for at least 25-30 hours per week)

  1. Proof of current ARRT certification (if applicable)
  2. Four letters of recommendation (submitted with application, they must be in sealed envelopes with the signature of the reference across the seal)
  3. Proof of current OJT position and letter stating the number of hours per week in this position (Required at least 25-30 hrs/wk). A signed letter from the administrator/manager stating the specific job description and number of hours working per week demonstrates proof.
  4. Completed & Signed Clinical Site Support Form.
