MS Degree Program: Application Forms
- Submit a UW-La Crosse Graduate application. This can be completed online at the following address: https://apply.wisconsin.edu. Select Graduate Degree and Health Related Studies: Medical Dosimetry [SAH], where applicable.

Official transcripts of all college and university work including the non-refundable Graduate School Application fee of $56 payable to UWL must be submitted to:

- Submit the UW-L Medical Dosimetry Graduate Application and all supplemental materials together by February 1st for general admission including the non-refundable $50 program application fee payable to the University of Wisconsin - La Crosse to:
Note: Failure to receive your application materials by February 1st will result in your application not being reviewed.
All completed applicants will be notified via email within approximately five (5) working days of receiving the application.

Track A - Baccalaureate RTT's
- Proof of current ARRT certification
- Four letters of recommendation; (submitted with application; they must be in sealed envelopes with the signature of the reference across the seal)
- Observation Rotation: all applicants must have completed a 40 hour observation in medical dosimetry. Read the observation information and associated forms that are due as part of the application process: Clinical Observation Forms

Track B - Baccalaureate non-RTT's
- Four letters of recommendation (submitted with application, they must be in sealed envelopes with the signature of the reference across the seal)
