MS Degree Program: Application Forms

  1. Submit a UW-La Crosse Graduate application. This can be completed online at the following address: https://apply.wisconsin.edu.  Select Graduate Degree and Health Related Studies: Medical Dosimetry [SAH], where applicable.
  2. Official transcripts of all college and university work including the non-refundable Graduate School Application fee of $56 payable to UWL must be submitted to:

  1. Submit the UW-L Medical Dosimetry Graduate Application and all supplemental materials together by February 1st for general admission including the non-refundable $50  program application fee payable to the University of Wisconsin - La Crosse to:

Note: Failure to receive your application materials by February 1st will result in your application not being reviewed.

Track A - Baccalaureate RTT's

  1. Proof of current ARRT certification
  2. Four letters of recommendation; (submitted with application; they must be in sealed envelopes with the signature of the reference across the seal)
  3. Observation Rotation: all applicants must have completed a 40 hour observation in medical dosimetry. Read the observation information and associated forms that are due as part of the application process: Clinical Observation Forms
  4. Application Checklist

Track B - Baccalaureate non-RTT's

  1. Four letters of recommendation (submitted with application, they must be in sealed envelopes with the signature of the reference across the seal)
