MS Degree Completion Program: Application Forms
- Submit a UW-La Crosse Graduate application. This can be completed online at the following address: https://apply.wisconsin.edu. Select Graduate Degree and Health Related Studies: Medical Dosimetry [SAH], where applicable.

Official transcripts of all college and university work including the non-refundable Graduate School Application fee of $56 payable to UWL must be submitted to:

- Submit the UW-L Medical Dosimetry Degree Completion Application and all supplemental materials together by the specified dates below including the non-refundable $50 program application fee payable to the University of Wisconsin - La Crosse to:
Application Deadlines:
February 1st for Summer Semester
June 1st for Fall Semester
Note: Failure to receive your application materials by the application deadlines will result in your application not being reviewed.
All completed applicants will be notified via email within approximately five (5) working days of receiving the application.
