To apply for graduate international admission, please follow the steps below:
Step 1: Complete a UW-L Graduate Application Form or On-line or Request a paper application packet to be sent to you by postal mail.

For Doctor of Physical Therapy program, in addition to the graduate application form, there are supplemental forms such as cover letter, clinical evaluation, etc.  Please click here for detailed information.        
Step 2: Complete a UW-L Background Information Form.
Step 3: Submit official copies with literal English translation of academic credentials (grade reports, transcripts or mark sheets) in previous higher education institution (college or university).  A professional credential evaluation from the Educational Credential Evaluators (www.ece.org) or World Education Services (www.wes.org) is required for all Physical Therapy program applicants who have a degree from outside of the U.S.
Step 4: Have an official TOEFL score report sent directly by ETS--for all applicants whose native language is not English.  The minimum required score is 550 PBT/213 CBT/79 iBT. UW-La Crosse institution code is 1914. An official IELTS score is acceptable.  The minimum required score is 6.0.   For Doctor of Physical Therapy program, the minimum required TOEFL score is 600 PBT/250 CBT/100 iBT or a 6.5 on IELTS.
  • Applicants who do not have a TOEFL or IELTS score or whose TOEFL score is below 550 PBT/213 CBT/79 iBT or IELTS score below 6.0, will be considered for conditional admission.  For detailed information, please click here.
  • Applicants who have earned a bachelor's degree from an English-speaking country (click here) are exempt from the TOEFL requirement.
Step 5: An Official GRE or GMAT (for MBA program) score report is required by some graduate programs at UW-L and is recommended for others.  Please refer to your graduate program information regarding the GRE/GMAT and other specific program requirements.  A photocopy should be submitted with the university application materials.
Step 6: Complete a Certification of Financial Resources Form with a supporting bank statement and a letter of support.
Step 7: Submit an Application Fee ($56 USD) and Processing Fee ($50 USD).  Both fees are required and non-refundable and should be made payable in U.S. dollars to the “University of Wisconsin-La Crosse”.  Please submit both fees in two payments by money orders, bank drafts, or U.S. checks.
Step 8: Secure housing: International Student Residence Hall Reservation Form and Contract Acceptance

Applicants will receive an email confirmation of their application packet and information regarding their admission status. When all required items have been received, an acceptance decision will be made.  Students who meet all requirements for acceptance to the university will receive a letter of acceptance, proper immigration papers, and travel information by regular mail. Admission and immigration documents will be sent by express mail service if payment of $40 USD (payable to UW-La Crosse) is submitted for this service.
