Application Forms

The study abroad guidelines, application and forms are available as Word files.

ALL students must submit items #1 & #2.  Students applying to: API, CEA, CIEE, Escape, GlobaLinks, ISEP, Pacific Challenge, UW-Eau Claire, and UW-Platteville programs should disregard items #3-6 and instead complete the application process for the respective program.

For students applying to any program NOT listed above, please submit ALL items.

  1. Application Form (use your tab and arrow keys to navigate the form)

  2. $50 application fee (payable to UW-La Crosse)

  3. Official University Transcripts

  4. Academic Recommendation Form (print 2)

    5. One letter must be from a CURRENT instructor (within the past academic year).

    International Students must submit an approval note from the OIE International Student Advisor in addition to the two letters of recommendation.

  5. Personal Statement Form

  6. Foreign Language Essay Form (complete only if you are applying to a program which requires a foreign language).
