Requirements for a Complete File To be considered complete an applicant's file must meet the following requirements: 1.  The CASPA application must be complete with all transcripts and all 3 references received by CASPA by the September 1, 2008. 2.  The UW-L - Gundersen - Mayo PA Program Supplemental Application must be in the Health Profession Admissions office by the deadline. 3.  The official GRE (University of Wisconsin – La Crosse Institution Code 1914 and Medical Sciences Program Code 0609) score report must be received by UW-L by the deadline. 4.  Any additional required documentation such as international transcript reviews, TOEFL and TSE scores must be received at UW-L by the deadline.
