Grants Final Report Form
The University of Wisconsin-La Crosse uses a standardized Grants - Final Reporting Form to report on internal grant funds. The final report due date is the third Friday of February annually.
If you have received a grant from UW-L, (e.g., research, professional development, international development, etc.) you should use this form to report on your grant project. After you complete and submit your report you will receive a copy of it via email for your own records.
NOTE: You will need to complete the form in one sitting. The software does not allow you to “save” your work and return to it later. Nor, does it have spell check capabilities. If your responses are lengthy, you may find it best to write them in a word processing program and then cut and paste into the report form.

- Academic Staff Professional Development
- Faculty Development Grants
- Fund for Innovation in General Education
- Faculty Research Grants
- International Development Grants
Directions for Use:
The Grants - Final Reporting Form will take you directly to the SelectSurvey.net sign in page for completion of the final report for internal grants. Users should sign in using their normal 8.4 username, which is last name (8 characters) . and first four of first name). (i.e., beauty.slee for Sleeping Beauty), using the same password used for sign in to Outlook. The word "eagle" does NOT need to be used as it is already displayed on the log-in screen.
