Elgibility:

Must be a current Franciscan Skemp patient (Onalaska clinic) living in Onalaska, Holmen or West Salem.

Cost:

Free to patients

* Patient can obtain a free coupon from the Patient Liaision to give to the driver.

   Coupon may be acquired either before scheduling the ride or upon arrival to the clinic.

Hours/Days of Operation:

7:00 AM - 9 PM (Monday - Friday)

9:00 AM - 5 PM (Saturday - Sunday)Onalaska Clinic

Appointment Instructions:

- Must call ahead for reservations (ask for Wheelchair accessible van).

- Prefers that appointments are made only a few hours before requested pickup time.
