Student Information and Enrollment Verification Form

Students who need enrollment verification for Health Insurance, Loan Deferments, Scholarships, etc. should now print out a formal statement of attendance by clicking the "Enrollment Verification" menu item under the Enrollment Resources menu located on the lower right side of your WINGS student center which will take you to a secure connection to the National Student Clearinghouse. This is a free service to our students.

The following information is intended for employers, credit card  companies, scholarships, insurance companies, third parties and other agencies needing to verify student enrollment at UW-La Crosse.

This site is intended to provide agencies instructions on how to obtain enrollment verification. Enrollment verification includes the following: 

  • Status - full or part time enrollment
  • Dates of attendance

You can also obtain a Good Student Discount Certificate from this secure site.

The University of Wisconsin-La Crosse has authorized the National Student Clearinghouse (NSC) to act as our agent for all verifications of student enrollment NSC may be contacted at:

Verifications are available 24 hours a day, 7 days a week, 365 days a year via the Clearinghouse web site.

Please note:  Non-directory information cannot be released without written authorization from the student. This includes GPA or good student discount verification.
