School Health Education 910 Add-on Program
About the Program
The University of Wisconsin-La Crosse is offering a 15 credit program for people seeking the Wisconsin Department of Public Instruction 910 Add-On Health License. Interested participants must hold a current, Wisconsin DPI teaching license. This program is approved for licensure in the State of Wisconsin only and is not intended for licensure in any other state.
This program is designed to better serve the needs of professionals who are unable to come to campus during the regular school year. It will combine traditional on-campus learning experiences with on-line coursework so that participants may complete a portion of the program from home. An outline of the required coursework and the sequence of experiences are listed below:
Spring 2011 Semester
- SHE 705 – Essentials of Health & Wellness (3 credits, on-line format)
- Students may begin on: March 1 or April 1
Summer 2011 Semester (9 credits, hybrid format)
- SHE 710 – Leadership in Health Education (3 credits)
- SHE 715 – Curriculum & Instructional Methodology (3 credits)
- SHE 720 – Adolescent Health Issues (3 credits)
- Students meet in La Crosse on: June 20-23, July 11-14, and August 1-4
Fall 2011 Semester
- HED 725 – Seminar / Field Experience (3 credits, hybrid format)
- Students must meet on campus December 3, 4, & 5
In order to earn the 910 license, participants must complete the entire program in the sequence listed above and successfully pass the Praxis II examination.

Application Procedures
Step 1
The following are required for admission to the SHE 910 Add-on Licensure Program:
- application form (FYI: This is a link to a single-page electronic application form);
- a letter of interest explaining your goals for graduate work in school health;
- a 3.0 cumulative grade point average (gpa) is preferred (candidates with a lower gpa may be considered for admission if space is available);
- official transcripts (if you are currently enrolled in another program at UWL, you may send an unofficial transcript); and
- a copy of your current teaching license; (NOTE: if you are currently student teaching, then you will need an official letter stating that you will successfully complete all graduation requirements before June 20, 2011.)

All complete applications received by January 7, 2011, will receive full consideration for admission. Applications received or completed after that date will be considered on a space-available basis.
Step 2
After being accepted into the SHE 910 Program, candidates must then apply online for admission to the University of Wisconsin-La Crosse. There are two options listed below:
- Graduate Non-degree/Special Students (see http://www.uwlax.edu/admissions/html/gradmis.htm)
- MS degree seeking students will complete the UW-La Crosse Graduate Studies application form (there is a $56 application fee, see http://www.uwlax.edu/admissions/html/gradmis.htm).

PRAXIS II test information
Candidates may take the subject assessment after they have completed SHE 705 Essential of Health and Wellness. Official passing scores must be received by the UW-La Crosse Teacher Certification Office prior to seeking licensure in health education.

Preparing for the Test

- Satisfactory completion of all Health Education content coursework is recommended prior to taking the test
- Work through a PAXIS II Study Manual—available from most booksellers and at Murphy Library
- Receive tutoring from a counselor—contact the UW-L Counseling and Testing Center
Official Scores
Scores are only considered official if they are received in the Teacher Certification Office, 145 Graff Main Hall directly from the Education Testing Service (ETS).

- PRAXIS II score verification is recorded on the UW-La Crosse computer record system as scores are received from the Educational Testing Service (ETS).

Licensure process
