Motor Development Program Application Materials

Prospective Motor Development Program Participants

Thank you for your interest in the Motor Development Program at UW-La Crosse. Enclosed you will find program information and registration forms that are necessary to enroll an individual in this program.
To enroll an individual in the program, please follow the procedures specified below.
- Complete the following forms and return them to the Special Populations Programs office at UW-La Crosse (do not send any program fees at this time). Failure to fully complete these forms may delay admittance to the Motor Development Program.
- Complete and sign the
Medical Release Form and then forward the following to the
prospective participant’s appropriate physician.
- Medical Release Form (completed and signed by parent/guardian)
- Medical Information Form (scroll down to form; to be completed by physician)
- Complete and sign the
School Information Release Form and forward the following to the
prospective participant’s appropriate school teacher or other school
personnel.
- School Information Release Form (completed and signed by parent/guardian)
- Confidential School Information Form (scroll down to form; to be completed by school personnel)
Upon receipt of all completed information and if space is available for new participants, we will notify you of acceptance. If space is not available, we will place the prospective participant on a waiting list.
