Student Teaching, Internship or Practicum Application Information
Required of All Student Teacher/Intern and Practicum (EC & SPE) Applicants:
April 26 is Application Due Day to student teach or intern during Spring 2008. Please complete online forms as available and print out. Turn in your completed application packet at CC 259 on April 26 anytime from 9:30 a.m. to 1:30 p.m. You may also turn in your Hepatitis B immunization record with your application packet. Plan to have your TB test done in December and bring in or send in your results by December 21, 2007.
You will receive directions for the online criminal background check procedure at the Information Day session. The CBC will cost anywhere from $24 to $45, depending on individual circumstances. Also read the Responsibilities of Student Teachers/Interns.

The one-page Checklist below is turned in as the cover sheet for your application packet. Use the Checklist as a guide to determine which forms you need to fill out. Copies of these forms and other SOE forms are available in the forms section.
Download the DOC verisons of the forms if you'd like to type directly onto the form, print it and turn it in. If you anticipate filling out the forms by hand, the PDF versions are recommended.
