NOTE: A copy of your UW-L unofficial transcript (including the '08 fall semester) available through TALON should be attached to each application.
No letters of recommendation are required unless otherwise stated in scholarship criteria.
Completed packets (stapled in the upper left hand corner) should be returned to the assigned area listed in the Foundation Web site on or before March 1. A completed packet includes this application and your transcript and letters of recommendation where requested. Incomplete applications or applications not meeting the criteria will not be considered. Questions should be directed to Sara Olson at 785-8491.
