Community Health Education Portfolios
A CHE template has been created for you to start your portfolio. Only download this template if you are starting a brand new portfolio. DO NOT download if you have an existing portfolio.
Here is a sample portfolio. To begin your portfolio, download this template to your online files or personal computer.
All CHE students have seven responsibilities to complete before graduation. Each responsibility also has sub-competencies that must be displayed. TLC has organized these responsibilities into a table that allows students to create direct links to caption sheets and artifacts. Caption sheets provide a description of what the student has done to meet the proper criteria. If you need a copy of the responsibility page or caption sheet, right click on the files listed below to save or open the pages and copy the content to your portfolio space.
Selecting Artifacts:
When selecting your artifacts make sure they show evidence of completing your standards. Ask yourself the following questions:
- How does this artifact represent the standard/competency?
- Is this artifact the best representation of my teaching?
- How and why was this artifact meaningful?
- What does this artifact communicate about my teaching?
Types of Artifacts:
There are several types of artifacts. All types are discussed on our artifact page.
Caption Sheets:
Caption sheets must accompany each artifact you use to meet a
competency. These sheets are an electronic form that you fill in with
information about your artifact. After you fill in the form you save it
and create a hyperlink within your responsibilities table to that page.
For organizational purposes it is important to create a folder that
contains all of your caption sheets. It is even more important to
properly name these caption sheets for the competency they are related
to. For example: 1_a would be an appropriate name for the caption sheet
that is attached to responsibilities 1 competency A.
