Fee Schedule  

 UWL
  • Undergraduate or Graduate students currently enrolled in ≥ 1 credits do not have to pay additional segregated fees to use the Student Health Center (SHC)
  • There is a service fee for students enrolled in summer courses of $8.39 per credit or $50.33 for ≥6 credits
 WTC
  • Students enrolled in ≥ 6 credits do not have to pay an additional segregated fee to use the SHC
    • There is an additional charge of $10 for EACH office visit billed to student's account
    • There is an additional charge of $10 for 15 Physical Therapy visits billed to student's account
  • Students enrolled in < 6 credits must pay fee (currently ~$100 for spring or fall semester or ~$50 for summer)
UW MADISON RN STUDENTS
  • Do not have to pay additional segregated fee to use the SHC
  • Must pay cash at time of service for any additional services or equipment
 UW MADISON PHARMACY STUDENTS
  • Must pay segregated fee in cash to use the SHC (currently $90.80/semester)
 AMITY STUDENTS
  • Do not have to pay additional segregated fee to use the SHC
  • Must pay cash at time of service for any additional services or equipment
