UW-L Upward Bound Graduate Survey (2009-2010)

UB Grads - as you know, we must report on how well our graduates are meeting our goals - getting in to college, staying in college and graduating from college.

The Department of Education requires an Annual Performance Report (APR) in the fall of the year. The more accurate our information is, the better.

So, in October, we mail a graduate survey to selected grads, and we hope that all of you will take a few minutes to complete this page.

By the way, if you were in college but had to leave for some reason (money, grades, family issues, etc.) and would like to return, PLEASE CONTACT US! We can help you explore your options and get back on the college track!

We believe our APR will be due in December. Please try to complete this form by November 1 if possible (but any time will help.)
