PRAXIS II Health Education Content Knowledge Test (#5551)

Evidence of passing the Praxis II Health Education content knowledge test (#5551) is required for program completion. A score of 151 or higher is required to satisfy the Praxis II testing requirement. Designate UW-La Crosse as a score recipient when registering to test to ensure receipt of test results by UW-La Crosse. Register to test at www.ets.org/praxis. The Testing Center at UW-La Crosse is a computer testing site. Score results are usually available to UW-La Crosse three-four weeks after testing.

Applying for a Health Education License (Wisconsin only)

Successful completion of all course work, portfolio, and Praxis II testing is required to apply for a health education license. Once UW-La Crosse has verified and recorded program completion on the UW-La Crosse transcript, the teacher certification officer will submit program completion information to the DPI.

The health education license is added using the Educator Licensing Online system (ELO).  Select the applicant type "First-time request for a teacher license or for adding a new subject/level to an existing teaching license"  from:
