The following is required for admission to the School Health Education 910 Add-on Licensure Program:

  • A required 3.0 cumulative grade point average (GPA); candidates with a lower GPA may be considered for admission with permission from the director.
  • You must have a current Wisconsin teaching license.

To apply to our School Health Education 910 Program:

Step 1:
Submit Statement of Intent to the Director at
Deadline for submitting letter of intent: January 18, 2019.

Students will be asked to submit the following information for review by the director:

  • A letter of interest explaining your reasons for applying to our program as well as your goals within the school health education field.
  • An updated resume.

Students will be invited to apply for admission to UW-La Crosse once their Statement of Intent has been received, reviewed and accepted by the director.  Students will be notified when that occurs and they may then submit the online application for admission to UW-La Crosse. 

Step 2:
Admission Application to UW-La Crosse.
Deadline for submitting your application: January 25, 2019.
