The following forms need to be completed by all new employees and all returning employees who have not been paid on the UW-La Crosse payroll within the past 12 months.

UW-La Crosse uses an electronic I-9 form. New employees will be asked to complete the electronic form and present acceptable documents to the Human Resources office.

Other important information

The Affordable Care Act (ACA) requires most everyone to obtain health insurance for themselves and their dependents or pay a penalty when filling tax returns.  The health insurance marketplace is a new option for people to obtain health insurance. UW-La Crosse will provide the required health insurance marketplace notice to all employees at the time of I-9 completion.

Executive Order #54, issued by the Governor in 2011, requires all University of Wisconsin system employees to report child abuse and neglect if, in the course of employment, a UW System employee observes or learns of such an incident. UW-La Crosse has prepared for employees and their supervisors guidance in complying with Executive Order 54.
