The following forms need to be completed by all new employees and all returning employees who have not been paid on the UW-La Crosse payroll within the past 12 months.
Forms to complete

Note: The I-9 form requires employees provide appropriate original identification prior to the employment start date. Examples of acceptable documentation include passport, or photo ID AND social security card, or photo ID AND birth certificate (photo copies not accepted -- see full list on last page of form I-9). A copy of the social security card will also be required, if the original is not used for the I-9 employment eligibility verification.
Employees who work remotely and are unable to complete form I-9 with a Human Resources staff member at UWL have the option of completing the form with a notary or authorized representative. If that is the case, please use the instructions for I-9 completion by a notary or authorized representative.
Other important information
The Affordable Care Act (ACA) requires most everyone to obtain health insurance for themselves and their dependents or pay a penalty when filling tax returns. The health insurance marketplace is a new option for people to obtain health insurance. UW-La Crosse will provide the required health insurance marketplace notice to all employees at the time of I-9 completion.
Executive Order #54, issued by the Governor in 2011, requires all University of Wisconsin system employees to report child abuse and neglect if, in the course of employment, a UW System employee observes or learns of such an incident. UW-La Crosse has prepared for employees and their supervisors guidance in complying with Executive Order 54.
