The Physical Therapy Department now offers
its application materials online in Adobe® Reader ® (PDF) format.
You will need to have the free Acrobat Reader software installed
on your computer to view and print these files. If you need the
software, you can
download it free from Adobe. Below is a list of available
application documents in Acrobat (.pdf) format:

All students (except International Applicants) must submit all application materials together (below) to the Physical Therapy program in addition to the graduate school application to UW-La Crosse.

Correct mailing addresses for application materials, including payments, can be found on either the Application Instructions form or the Application Checklist.
In addition to the above application materials required by the PT Program, applicants must also apply for admission to the University of Wisconsin-La Crosse School of Graduate Studies. Applications to the University of Wisconsin-La Crosse Graduate School are now available online here.
Regarding UW-L transcripts, current UW-L students do not need to forward transcripts with their application. Records and Registration will automatically pull your transcript when they receive your graduate school application. UW-L graduates and students who have transferred from UW-L may want to contact Records and Registration for further UW-L transcript information. All other transcripts should be included with your graduate school application.
