University of Wisconsin-La Crosse Doctor of Physical Therapy ADMISSIONS PROCEDURE: It is the policy of the University of Wisconsin Physical Therapy Consortium: UW-L COHORT that all students who are desire admission to the program must meet the requirements and complete the admission process. All applications will be carefully reviewed, adhering to the procedure detailed below. Overview of the Admissions Process

1. degree will be completed by May 30th of application year 2. applicant is enrolled in the University of Wisconsin – La Crosse: Biology – Physical Therapy Dual Degree Program Physics – Physical Therapy Dual Degree Program

All students (except International Applicants) must all application materials together (below) to the Physical Therapy program in addition to the graduate school application to UW-La Crosse.
International Applicants Complete applications are due in the Office of International Education by October 1st for early admission and December 15th for general admission. Applicants must adhere to the following application instructions. In addition to the Physical Therapy program application and the university application the following items are required: Test of Spoken English (TSE) - minimum score of 45; TOEFL - minimum score of 600 (250 CBT), review of degree/coursework from outside of the United States by a professional credential evaluation service such as Education Credential Evaluators, Inc. (www.ece.org). International Applicants must use the university application materials issued by the UW-L Office of International Education. These materials may be printed from the website at www.uwlax.edu/oie or both the physical therapy application packet and university paper application packets may be requested from uwlworld@uwlax.edu The physical therapy application packet, the university application materials, and the items listed above should be submitted to University of Wisconsin-La Crosse, Office of International Education (International Admissions), 116 Graff Main Hall, 1725 State Street, La Crosse, WI 54601 USA. |
