Report an Incident or Concern

A page within Student Life

UWL's Mission states the University "provides a challenging, dynamic, and diverse learning environment in which the entire university community is full engaged in supporting student success."

Active communication about incidents on campus, student concerns, and misconduct supports student success and ensures a safe educational environment. This page offers the different ways that staff, faculty, student, families, and parents can report incidents or concerns to appropriate university officials.

Report Non-Academic or Academic Misconduct

Hate and Bias Report

A hate/bias incident is any physically or verbally harmful act that is motivated by (or appears to be motivated by, in whole or in part) any of the following factors: race/ethnicity, religion, national origin/immigration/citizenship status, sex, sexual orientation, gender identity or expression, economic status, language, age, size, pregnancy or family/marital status, disability, veteran or military status, or any other identity status protected by University policy.

Student of Concern Report

UWL is committed a campus community dedicated to supporting student success. We recognize that some students may face life events or personal difficulties that require more intentional coordination and support. To that end, we ask all campus community members to report any concerning behaviors or refer students who may need this intentional support by using this form.

Academic Misconduct

The University and its faculty, staff, and students believe that academic honesty and integrity are fundamental to the mission of higher education and the University of Wisconsin La Crosse.  The University has a responsibility to promote academic honesty and integrity and to develop procedures to deal effectively with instances of academic dishonesty. UWS Chapter 14 governs policies and student rights regarding Academic Misconduct.

See Student Life's Academic Misconduct Guide for Instructors

Non-Academic Misconduct

In promoting such environments, the University has a responsibility to address student nonacademic misconduct; this responsibility is separate from and independent of any civil or criminal action resulting from a student’s conduct.  The University of Wisconsin La Crosse is committed to respecting students’ constitutional rights. UWS Chapter 17 governs Non-Academic Misconduct at UWL. UWS Chapter 18 provides information about conduct on university lands.

Please see UWL's Non-Academic Misconduct webpage

Sexual Misconduct Reporting Form

University employees (other than designated confidential resources) are required to report any incidents of sexual violence they witness or learn of, and to provide all relevant information.
