Employee Action Form (EAF) Update

EAF Change

The Office of Human Resources will be replacing the ‘paper’ version of the Employee Action Form, (EAF) with an online form beginning September 1, 2023.

The transition to the online form migrates the current form process from a PDF form submission via email and tracking in the PeopleAdmin system, to an all-in-one program (BP Logix) environment that allows for you to modify, terminate, and track your EAF requests. 

The benefits of the new form allow for:

  • A better-managed experience for submitters and approvers.
  • All-in-one submissions and tracking.
  • Automated communications and reminders.

The new online form includes more actions/reasons to better fit the changes to employee records. All actions/reasons selected have logic in place for required fields.

Once you have ‘submitted’ your request, you will be notified via email and can track your request's status in the BP Logix environment.

The new process will provide you with more email communications on the status of your employees and monitor the approval chain. Each approver has 9 days to approve any tasks in their EAF Task List. If there is no action by the approvers within 9 days, the process will automatically terminate, and a new request will need to be resubmitted.
