Completing the Health Assessment is a quick and easy first step toward tracking your well-being and making progress toward your Well Wisconsin $150* prepaid card. The Health Assessment is a 15-minute questionnaire that gives you a personalized report on your health. It will also customize your Well Wisconsin homepage with tools to support your specific goals.
Bonus: Finish your Health Assessment by September 15 to be entered into a drawing to receive a well-being prize in the mail!
What happens next?
- At the end of the Health Assessment, you’ll choose a time that works best for a health coach to reach out to you by phone. Use it as an opportunity to receive personalized guidance from a coach on your Health Assessment results.
