Academic & General Disability Documentation at UWL

In order to provide reasonable academic accommodations for a student, the ACCESS Center requires documentation that establishes a diagnosis of a disability and describes how the condition directly and substantially limits a major life function for the student. Students may submit a letter or diagnostic report from their provider that contains the information about their disability and information about any limitations of major life activities. 

Documentation must be from an appropriate professional who has diagnosed the student's disability or who has sufficient history with the student to support a previous diagnosis. UWL will consider IEPs and 504 Plans as documentation. 

Students whose documentation is incomplete or insufficient may be eligible for provisional accommodations (reassessed every semester) while working on obtaining further information from their doctor or mental health care provider.

Students must register with the ACCESS Center to begin the process of requesting accommodations. Registration can be found here

Students and healthcare providers can use our Verification Form below to provide needed documentation.

For all housing accommodations, with the exception of Emotional Support Animals*, students may complete the Reasonable Housing Accommodation Request Form. The first half of the form is to be completed by the student and the second half of the form can be completed by a healthcare provider.

*Students should meet with an ACCESS Center Advisor to discuss the Emotional Support Animal policy, procedures, and documentation.
