All employee separation notifications MUST be submitted through Workday.  

Exiting Employee Form

Employee Separation Notification is an important and critical step in the offboarding process.  For separating employees, supervisors should follow these steps:

  1. The supervisor receives (or has documented) separation reasons for the employee. For example:
    1. Resignation letter/email
    2. Retirement letter/email
    3. Notice of separation
    4. Disciplinary document
    5. Notice of non-renewal 
  2. The supervisor completes the form below.
  3. Supervisor uploads document referenced in Step 1. 

Once this form is submitted, supervisors can expect the following:

  • HR is notified and begins the offboarding process.
  • The supervisor can expect a receipt of their submission.
  • The employee is notified of this submission and a note informing them of the exit process is provided.  
  • Need to know campus stakeholders are notified of an upcoming separation, e.g. Business Services, Purchasing, Parking, etc.
